Select the software product that is causing problems.
Enter a version number for the product. For example, you could enter 5.0.6.3 if you are using that version of the software.
How would you describe the severity of the problem to the core function of the software? Critical, severe, major, moderate, minor, difficult, or easy?
What operating system are you using? What version of the operating system? Is this running on a terminal server? An example of a good answer is Windows XP Service Pack 3.
When trying, how often are you able to reproduce this problem?
Please give a short summary of the problem.
Give a detailed description of the problem. If it's a connection issue, give details of the type of database being used, and let us know how you configured the database connection. Give any and every detail to that we will need to reproduce the problem.
Upload files for us to use to recreate the problem. If you have multiple files to upload, zip them into one file. We would like to see example reports used, screen shots, and anything else you think can help us.
